Job summary

Trillium Health Resources is seeking a Care Transitions Specialist II to support individuals transitioning between care settings through coordinated planning. The role develops transition plans, aligns services across providers and community partners, and addresses barriers to ensure stable transitions.

The position requires a RN licensure with experience or a relevant bachelor’s/master’s degree, a valid driver’s license, and NC residency within 75 miles of Duplin County.

Qualifications

• Requires Bachelor’s degree in a human services field or RN licensure with 3 years of relevant experience with SED/SMI; OR Master’s degree with 1 year of relevant experience.
• Valid driver’s license required.
• Must reside in NC within 75 miles of Duplin County and be legally eligible to work in the United States.

Responsibilities

• Coordinate and manage care transition activities across settings to support successful discharges and service changes.
• Develop and implement transition plans based on individual needs.
• Collaborate with providers and partners to align services during transition periods.
• Monitor transitions and intervene to resolve delays or barriers.
• Facilitate service connections and ensure continuity following transitions.
