Job summary

Trillium Health Resources in North Carolina seeks a Care Transitions Care Manager to coordinate care transitions for individuals in the My Choice program for IDD. The role develops care plans, assesses needs, and connects with providers to promote continuity and improved outcomes. Remote work within the Mid State/Southern regions is available.

You will educate families, link to community resources, and support sustained participation in care plans, with travel as required to meet program goals.

Qualifications

• RN license in NC with four years of MH/DD/SA experience.
• Graduate degree (Master’s) in a human service field preferred; 1 year MH/DD/SA experience.
• Bachelor’s in human services with 2 years MH/DD/SA experience.
• Bachelor’s in non-human services with 4 years MH/DD/SA experience.

Responsibilities

• Manage ongoing care coordination for assigned individuals across settings.
• Coordinate services across providers and community resources for LTSS and HCBS.
• Educate individuals and families about services and resources in the community.
• Link individuals to community resources for integration and participation in care plans.
• Travel within catchment as required and ensure continuity of care.
