Job summary

Alliance Health is seeking a Transition Support Specialist I to join our remote team serving North Carolina. This full-time role coordinates care transitions, communicates with plans and providers, and ensures timely follow-ups while supporting members and families.

The ideal candidate holds a Bachelor's in Human Services or related field with 2–4 years of experience, and may pursue NACCM, NADD-Specialist, Health Education Specialist, or CBIS credentials.

Qualifications

• Bachelor's degree in Human Services with 2 years of direct behavioral/physical health experience.
• Or Bachelor's in a non-human services field with 4 years of relevant experience.
• Preferred: NACCM, NADD-Specialist, Health Education Specialist, and CBIS.

Responsibilities

• Coordinate with internal teams and external partners to ensure timely, informed, and seamless care transitions.
• Manage inbound and outbound communications related to transition planning, data accuracy, and care coordination.
• Proactively identify and resolve transition-related issues to minimize care gaps and enhance member outcomes.
• Complete Transition Summaries and warm hand-offs with identified entities.
