Job summary

Sprinter Health is seeking a care navigator to join our remote, full-time team. You will assess patients to identify barriers to care, educate on services and resources, and ensure follow-through on care plans.

You will document interactions in HIPAA-compliant systems and coordinate across care teams. Ideal candidates have 4+ years in care navigation or related fields, strong critical thinking, and the ability to work independently in a fast-paced environment.

Qualifications

• 4+ years of experience in care navigation, coordination, social work, call centers, or patient outreach.
