Job summary

Nolia Health is seeking a Care Navigator to join a transdisciplinary team providing virtual telehealth care for older adults and their families. You will support patients and caregivers with biopsychosocial planning, connect families to local resources, and coordinate with primary care and specialists to ensure continuity of care.

• 2+ years in healthcare or community setting preferred.
• Experience understanding needs of family caregivers and older adults is a plus.

Responsibilities

• Provide telehealth care navigation to patients and families.
• Coordinate with clinical team to plan biopsychosocial care.
• Connect families to local resources and supports.
• Educate caregivers on care planning and self-care.
