Job summary

One Call seeks an internal case management support specialist to ensure best continuum of care for injured workers. You will work with all business units to provide timely services at optimal cost, coordinating care with carriers and physicians until discharge.

The role emphasizes assessment, collaboration, and advocacy to meet clinical needs and optimize outcomes for injured workers. This position supports remote operations with cross-functional teamwork.

Qualifications

• Two years of experience or equivalent with One Call processes and education.
• RN, LPN, or MA degree with relevant background.
• Experience coordinating care and interacting with multiple stakeholders.

Responsibilities

• Manage and review work-related injuries, coordinating care plans and timelines.
• Conduct initial referrals calls to summarize needs and plan of care.
• Assess needs by interviews, reviewing docs, and planning evaluations.
• Set schedules, arrange transportation, and coordinate services.
• Monitor progress, advocate for services, and document updates.
• Collaborate with One Call departments to fulfill referrals and improve outcomes.
• Provide educational resources and support staff development.
• Assist in discharge planning and post-discharge follow-up.
• Contribute to process improvements and reporting of results.
