Job summary

One Call is seeking an internal case management support specialist to ensure top-tier continuum of care for injured workers. You will collaborate with all business units to deliver timely services while controlling costs according to clinical needs.

As a remote-first organization, you will coordinate with carriers and providers, monitor treatment plans, and assist in discharge planning. This role emphasizes patient-focused, multidisciplinary care and continuous process improvement.

Qualifications

• RN/LPN or MA with a minimum of two years experience or equivalent.
• Strong communication and documentation skills.
• Experience coordinating care and working with multiple departments.

Responsibilities

• Manage injured worker cases with planning, coordination, and evaluation.
• Conduct initial referrals calls outlining needs and care plans.
• Coordinate treatment objectives, schedules, and resources.
• Monitor progress, advocate for services, and assist with discharge planning.
• Collaborate with care teams and providers to optimize outcomes.
