Job summary

One Call is seeking an internal case management support role focused on ensuring the best continuum of care for injured workers. You will collaborate with multiple teams to coordinate services, monitor plans, and support case closures by carriers or physicians.

The role emphasizes remote-first work with strong emphasis on communication, organization, and adherence to care protocols. Training and development opportunities are provided within a collaborative environment.

Qualifications

• RN/LP/N or MA with at least two years of experience.
• Excellent customer service and communication skills.
• Ability to work independently and in a team.
• Knowledge of case management and injury care processes.

Responsibilities

• Manage work-related injury cases and coordinate care plans.
• Conduct initial referrals and outline plan of care.
• Coordinate transportation and appointments.
• Monitor progress and advocate for needed services.
• Prepare discharge and post-discharge resources.
