Job summary

One Call is seeking an internal case management support specialist to ensure seamless care coordination for injured workers. You will work across business units to provide timely services at optimal cost and help carriers close cases with physician discharge when appropriate.

The role emphasizes collaborating with nursing staff and other departments to tailor plans of care, monitor progress, and communicate with clients.

Qualifications

• RN/LPN or MA with a minimum of two years of experience.
• Knowledge of care coordination processes and ability to coordinate services.
• Excellent written and verbal communication; strong organization.

Responsibilities

• Manage injured worker care plans and coordination.
• Conduct initial referrals with plan of care overview.
• Coordinate transportation and escorts as needed.
• Document and review medical records and progress.
• Collaborate with departments for referrals and discharge planning.
