UnitedHealth Group is seeking a Telephonic Care Navigator to support DSNP/CSNP members remotely in Washington or Oregon. The role involves outreach, care gap closure, scheduling, and resource linkage, with a focus on high-quality customer service.

The position is full-time, with 8-hour shifts, training, and remote work flexibility. Candidates should have healthcare/insurance experience, strong communication skills, and access to a home office.

Qualifications

• High School Diploma / GED or equivalent work experience.
• Must be 18 years of age or older.
• 1+ years of call center and/or telephonic customer service experience.
• 1+ years of professional office experience using telephone and computer as main tools.
• 1+ years of Healthcare/insurance or social work/community outreach experience.
• 1+ years of analyzing and solving customer problems.
• Experience using Word, Excel, and Outlook.
• Ability to work from home with high-speed internet.
• Willingness to attend 4 weeks of training.
• Full-time availability Monday–Friday with 8-hour shifts.

Responsabilités

• Completes telephonic outreach to DSNP/CSNP members via multiple outreach modes.
• Educates members on care gaps and helps schedule provider visits.
• Assists with social determinants of health and connects to community resources.
• Ensures access to PCP for members.
• Outreaches members per program guidelines.
• Meets quality and performance metrics.
• Provides excellent customer service to members and providers.
• Maintains schedule adherence and good attendance.
• Maintains confidentiality per HIPAA and regulations.
