Qualifications

• Associate’s degree in Healthcare or related field
• Minimum two years of experience in a directly related position in healthcare
• Proficiency with Microsoft Office and web-based technologies
• Basic Excel proficiency (pivot tables, formulas)
• Flexible schedule within assigned market hours
• BLS, CMA, CNA, EMT certifications preferred
• Fluent English preferred; bilingual a plus
• Knowledge of healthcare laws and patient privacy requirements

Responsibilities

• Conduct outbound calls to assess health needs and eligibility for care coordination programs
• Assist patients with scheduling appointments and navigating community resources
• Collaborate with patients, providers, referral coordinators and staff; document activities
• Communicate available care coordination services to patients and caregivers
• Screen for physical and behavioral health needs and refer as appropriate
• Provide data to providers, patients and families for informed decision making
