Job summary

Imua Family Services is implementing the Kamaaina Credit for ALICE Households Pilot in Maui County. The Care team will deliver monthly, strengths-based care coordination, connecting participants to community resources and documenting outcomes in Relief Compass.

The role combines remote engagement with occasional in-person trainings and events. Candidates should have strong communication, cultural humility, and the ability to manage 80–100 participants, with trauma-informed care at the forefront.

Qualifications

• Strong communication and active listening required.
• Caseload management of 80–100 participants.
• Familiarity with Relief Compass and virtual tools.
• Attention to confidentiality and trauma-informed care.
• Ability to work independently in a remote setting.
• Experience in care coordination or social services preferred but not required.

Responsibilities

• Conduct one structured participant touchpoint per month (1 hour).
• Assist participants with monthly assessments and confirm completion.
• Discuss needs, goals, progress, and barriers; aid goal setting.
• Provide resource referrals and light follow-up on referrals.
• Document contacts, assessments, and outcomes in Relief Compass.
