Think of Us is hiring a Care Navigator to support TAY and kinship families in California through a four-stage care path. The role centers on sustained, relationship-based navigation, using ConnectMe for intake, follow-up, and data capture while centering lived experience in every interaction.

The position emphasizes trauma-informed, culturally responsive practice and continuous learning, with a focus on scalable systems change and digital navigation.”

Qualifications

• 3+ years of experience navigating child welfare or similar services.
• Trained in Motivational Interviewing and able to engage with diverse populations.
• Experience using technology-enabled intake and data-driven workflows.

Responsibilities

• Engagement: build trust with TAY and kinship families across multiple touchpoints.
• Assessment: conduct MI-informed needs assessments and document priorities.
• Support: manage caseloads and create personalized action plans with clients.
• Learning & Systems Intelligence: document insights to improve the platform and inform policy.
