Job summary

Ohana Pacific Health is hiring a Community Health Worker to support kūpuna on Maui. This on-site role focuses on transition of care, health education, chronic disease management, and social services, with care coordination across hospital, rehab, and community settings.

The position emphasizes compassionate, empathetic service, cultural relevance, and collaboration with advanced practice providers to improve access to care and health outcomes for Hawaii's aging population.

Qualifications

• Experience working with elder populations is preferred.
• Strong interpersonal and teaching skills for patient education.
• Ability to navigate care transitions and coordinate services.

Responsibilities

• Provide in-person and telephonic support to seniors and caregivers.
• Assist with care plans and goal setting with APPs.
• Facilitate access to community resources and follow-up services.
• Coordinate with diverse care teams to ensure continuity of care.
• Educate patients on care processes and reduction of anxiety.
