Job summary

Qualifications

• High School diploma or GED is required.
• 1–2 years of related care coordination or similar experience.

Responsibilities

• Provide member outreach by phone or home visits to discuss care plans.
• Coordinate care activities with providers and community resources.
• Answer inquiries related to care plan and services.
• Communicate with care managers and practitioners to facilitate services.
• Assist with service assessments or screenings and document member needs.
• Maintain member records in accordance with state/regulatory requirements.
• Follow standards of practice and contractual regulatory guidelines.
• Identify needs and refer to Care Manager or Disease Manager.
• Educate members on available benefits and resources.
• Perform other duties as assigned.
