Centene is seeking a Remote Care Coordinator to support care management activities for members in Delaware. You will engage members by phone or home visits, discuss care plans, coordinate activities with providers and community resources, and help maintain member records in compliance with regulations.

The role is fully remote with standard hours 8:00 a.m.–5:00 p.m. ET and occasional alternate shifts to cover monthly needs; prior Delaware market familiarity is preferred.

Formación

• Requires a High School diploma or GED.
• 1–2 years of related experience.
• Pay Range: $17.84 - $28.02 per hour

Responsabilidades

• Outreach to members by phone or home visits to discuss care plans and next steps.
• Coordinate care activities with providers and community resources.
• Assist with member inquiries and maintain care records per regulatory standards.
• Educate members about benefits and resources available.
• Follow policies and standards; perform other duties as assigned.
