The hybrid position requires residing in Delaware with an active RN license and in-person visits about two days per week, with the rest remote.

Qualifications

• Requires an RN license with active state licensure.
• Bachelor's degree in Nursing and 2–4 years of related experience preferred.
• Experience in case management or care coordination is advantageous.

Responsibilities

• Evaluate member needs and barriers to access, focusing on high-priority items.
• Develop ongoing care plans and coordinate with providers to address unmet needs.
• Identify problems and implement appropriate care management interventions.
• Coordinate between members, families, and care teams to ensure person-centered care.
• Provide ongoing follow-up, monitor status, and revise plans as needed.
• Offer education to members and families on disease processes, care options, and benefits.
