Job summary

Think of Us is seeking a Care Navigator to provide relationship-based direct services to TAY and kinship families in California through a remote, technology-enabled model. This role centers on sustained navigation, trauma-informed assessments, and connecting families to community resources, while building the platform’s intelligence through data and lived experience.

Qualifications

• 3+ years experience navigating child welfare systems or similar social services.
• MI training and bilingual Spanish preferred.
• Systems navigator with ability to move through complex, fragmented systems.
• Experience centering voices of people with lived experience in the work.
• Passion for driving systems change in fast-growing organizations.
• Data entry and documentation in case management or CRM systems.
• Comfort with technology-enabled tools and digital intake workflows.

Responsibilities

• Engage TAY and kinship families to build trust across phone, text, email and digital platforms.
• Foster warm referrals through California community partners and collaborative problem-solving.
• Use micro-cash grants to stabilize participation and capture structured data.
• Activate crisis support protocols when needed using ConnectMe.
• Conduct Motivational Interviewing-informed needs assessments with trauma-informed care.
• Document progress and flag systemic barriers to inform strategy and advocacy.
