Job summary

Mom's Meals is seeking a Care Navigator to conduct social care screenings, eligibility assessments, referral coordination, and telephonic care navigation for Medicaid members and related programs. Qualifications

• Minimum of two years' experience in a healthcare, social service, or behavioral health setting.
• Engage individuals with empathy and professionalism despite barriers.
• Maintain accurate documentation in compliance with regulatory and organizational requirements.
• Experience with in-person or telephonic client interactions.
• Excellent written and verbal communication; strong interpersonal skills.
• Strong organizational skills; manage multiple priorities.
• Proficiency with Excel and standard office technology.
• Attention to detail and ability to prioritize tasks.
• Ability to work independently with minimal supervision.

Responsibilities

• Conduct standardized health-related social needs screenings.
• Complete eligibility assessments and documentation for social care services.
• Initiate, monitor, and close referrals in designated referral platforms.
• Accurately document and record service activities and time spent.
• Perform follow-up to confirm service connection and address barriers.
• Educate clients regarding resources, benefits, and services.
• Coordinate with providers and community organizations to support care.
• Escalate complex cases to supervisory staff as appropriate.
• Maintain HIPAA and confidentiality compliance.
• Meet productivity, timeliness, documentation, and quality standards.
• Participate in ongoing training and quality improvement.
• Support reassessment and renewal activities as required.
