Job summary

South County Mental Health Center, Inc. seeks a compassionate Family Systems Navigator to join the Mobile Response Team.

This role guides families involved with child welfare through complex service networks and coordinates care across school, welfare, juvenile justice, and community partners. Ideal candidates will have a bachelor’s degree in a human services field and at least five years in children’s systems, with strong communication and collaboration skills to support families and prevent

Qualifications

• Bachelor’s degree in a human services-related field is required.
• Minimum five (5) years of experience working within children’s systems.
• Strong knowledge of Child Welfare, School Systems, and Juvenile Justice.
• Excellent communication, collaboration, and organizational skills.

Responsibilities

• Assist families involved with child welfare in navigating the child welfare system.
• Coordinate services across multiple systems to reduce fragmentation of care.
• Serve as a navigation resource for families and system partners.
• Collaborate with Child Welfare to assist foster parents in preventing placement disruptions.
• Develop and maintain collaborative relationships with system partners.
