Job summary

Ophelia Health, Inc. is seeking a Care Coordination & Navigation Specialist (CCNS) to support a virtual program serving patients across Pennsylvania, New Jersey, New York, and Delaware.

You will engage with patients, build individualized care plans, and document progress in the EMR while coordinating with a clinical team. You will manage a caseload, perform triage, and research resources to resolve barriers.

Qualifications

• 2+ years in care navigation, case management, community health, or patient-facing support in healthcare, preferably virtual or phone.
• Experience with SDOH navigation or delivering patient experience in multi-channel environments.
• Ability to learn community resources in PA, NJ, NY, or DE; strong EMR and tech skills.

Responsibilities

• Serve as first point of contact for inbound patient communication and triage.
• Schedule visits with clinical team and coordinate logistics.
• Maintain and update patient data across systems.
• Troubleshoot technology issues with our platforms.
• Provide empathetic, solution-focused support to patients.
