Qualifications

Responsibilities

• Identifies patients not meeting clinical goals or important quality metrics and arranges follow-up per protocol.
• Reviews and updates medication list and documents allergies in the EHR.
• Uses registry data and evidence to assist point-of-care decisions and supports quality improvements.
• Identifies variances in standards of care that could lead to errors or delays and helps solve problems.
• Establishes external relationships with hospitals and post-acute care facilities.
• Provides care navigation services to specialty populations and collaborates with LVHN Specialty Institutes.
