Job summary

CareSource is seeking a Care Guide to coordinate member care as part of the Care Coordination Team. You will engage with members in various settings to establish relationships and manage care needs while collaborating with caregivers and providers.

Role emphasizes documenting care activities, facilitating post-discharge planning, and addressing barriers to improve outcomes. Travel to member and provider sites is required as part of the role.

Qualifications

• Associate’s degree or equivalent years of relevant work experience required.
• Minimum of one year of clinical experience in nursing, social services, or healthcare field.
• Medicaid and/or Medicare managed care experience preferred.

Responsibilities

• Coordinate care with the interdisciplinary Care Coordination Team across settings.
• Serve as a single point of contact for care coordination when CME involvement is not present.
• Identify needs, gaps in care, and barriers, and help implement interventions.
• Educate members and stakeholders about treatment options and resources.
• Document activities and member responses per standards and CareSource policies.
