Qualifications

• High School or GED required.
• Two years of healthcare or related experience required.
• Proficient with Microsoft Office (Outlook, Word, Excel).
• Knowledge of Medicaid, Medicare and Managed Care preferred.
• Customer service focus with strong listening and verbal skills.
• Professional phone etiquette.
• Able to multitask using multiple computer applications.
• Strong organizational skills.
• Able to work independently and in a team.
• Understanding of managed care concepts.
• Adaptable to a changing environment.
• Able to type and talk simultaneously.

Responsabilités

• Serve as first point of contact for enrollees and assist with understanding processes.
• Conduits for outreach to members and providers to confirm service details.
• Coordinate services with long-term supports teams and document activities.
• Provide in-person visits at least annually with consent.
• Maintain accurate, timely documentation across systems for visibility.
• Coordinate scheduling, support high-volume outreach, ensure task completion.
• Identify social support or community-based needs.
• Lead the interdisciplinary care team for enrollee-centered plans.
• Educate on plan benefits and service options.
• Escalate concerns and coordinate responses promptly.
