Isaac Health in the United States is seeking a Care Navigator Assistant to provide virtual and administrative support to the Care Navigation team, streamline patient panels, and track engagement goals.

The ideal candidate is highly organized, data‑driven, and proficient with EHRs, digital fax, and spreadsheets; strong communication and empathy are essential as we coordinate care and resources for brain health patients.

Qualifications

• 1–3 years of administrative or virtual assistant experience in care management or neurology.
• Proficient with electronic health records (EHR), digital fax platforms, and tracking spreadsheets.
• Metric-driven with ability to meet outreach and billable time targets.
• Excellent written and verbal communication; empathetic with patients and partners.

Responsabilités

• Daily list management to ensure 100% panel outreach.
• Track progress toward check-in goals and partner milestones.
• Ensure 20 minutes of billable time per patient interaction.
• Manage inbox, records requests, and chart housekeeping in the EHR.
• Coordinate insurance eligibility and benefits verification.
• Send care plans, visit notes, and referrals as requested.
• Send reminders and locate local community resources.
• Provide just-in-time administrative support to Care Navigators.
