Job summary

Salma Health, Inc. is seeking a Care Coordinator to join our interdisciplinary team, supporting patients across brain health and mental health services.

You will assist with intake, screening, insurance navigation, and treatment coordination, ensuring compassionate, timely care. In a fast-paced clinical setting, you will collaborate with physicians, therapists, and care coordinators, manage EHR documentation, and guide patients from initial contact through follow-up.

Qualifications

• Compliance with state board requirements, including fingerprinting and application submission.
• Proficiency with telehealth platforms and remote clinical workflows.
• Strong interpersonal, communication, and documentation skills.
• Experience in mental health clinics, psychiatric settings, or medical offices with multidisciplinary collaboration.
• Experience in fast-paced call centers with multi-channel follow-up.
• ,
• Spanish language skills are a plus.

Responsibilities

• Conduct patient screening calls following structured protocols and apply sound judgment.
• Assist with intake form completion and onboarding processes.
• Help patients understand insurance coverage and eligibility.
• Document interactions in dashboards and EHR.
• Identify safety risks and escalate as needed.
• Collaborate with clinicians to connect patients to care pathways.
• Respond to inquiries via phone, secure messaging, and email.
• Support treatment planning and discharge processes with clinical teams.
• Coordinate outbound outreach between external partnerships and Salma Health.
• Schedule intake and follow-up appointments.
• Coordinate appointment rescheduling and related workflows.
