Sonnet Health seeks a Brain Health Navigator who guides patients and families from their first call through long-term planning. You’ll work closely with our clinical team, including a board-certified neurologist, to turn ideas into practical workflows and tools that feel delightful for patients.

This fully remote, part-time role offers growth as the startup expands; strong empathy, dementia knowledge, and comfort with ambiguity will help you thrive and shape how care navigation evolves.

Formación

• 3+ years of experience in brain health, dementia, or related care, or in patient navigation.
• Solid knowledge of aging, cognition, MCI, dementia, and risk factors.
• Genuine empathy with warmth and skill for phone and video conversations.
• RN or LCSW background is a plus.

Responsabilidades

• Run intake calls with new potential patients to understand goals.
• Walk individuals through Brain Health Report and build personal plans.
• Coordinate tests and evaluations ordered by the clinical team.
• Provide ongoing check-ins to keep patients on track.
• Coordinate with doctors, families, and caregivers; connect to resources.
• Escalate concerns to the clinical team and maintain records.
• Help shape navigation workflows as Sonnet grows.
