This full‑time, home‑based role requires Illinois residency, up to 75% travel to West Suburbs locations, and strong analytical, communication and problem‑solving skills with proficiency in MS Office and EHR systems.

Formación

• Must reside in Illinois
• Willing and able to travel up to 75% of the time from home location around West Suburbs IL
• Minimum 3–5 years clinical practical experience
• Independent thinker comfortable working at home
• Excellent analytical and problem‑solving skills
• Effective communication, organizational and interpersonal skills
• Ability to work independently
• Proficient with MS Word, Excel, Outlook, and PowerPoint; familiarity with proprietary applications

Responsabilidades

• Coordinate case management activities to support member outcomes
• Assist with appointment scheduling and benefits access
• Use care management tools to educate members
• Collaborate with members, providers and community organizations
• Travel as needed within West Suburbs IL; mileage reimbursed
