Job summary
Trillium Health Resources in North Carolina seeks a Waitlist Care Manager to provide Tailored Care Management to RUN members, coordinating physical and behavioral health needs and connecting them with available resources. This full-time, remote role supports engagement, needs assessment, and navigation of Medicaid and disability determination processes, with typical hours and work-from-home options.
Qualified professionals must have licensure or degrees as described, a valid driver’s license,
Qualifications
- Must meet Qualified Professional (QP) status.
- Fully licensed by NC governing board regulating Human Services professions; OR
- RN licensed in NC with MH/DD/SA experience; OR
- Master's degree in human services with MH/DD/SA experience; OR
- Bachelor's degree in human services with MH/DD/SA experience; OR
- Bachelor's in non-human-services with MH/DD/SA experience.
Responsibilities
- Provide Tailored Care Management to RUN participants.
- Review RUN applications for potential Innovations Waiver eligibility.
- Educate individuals/families on RUN details and available resources.
- Assist members in obtaining clinical information and starting Medicaid or disability determination processes.
