Job summary

Trillium Health Resources is seeking a Care Manager-RB-BHT to provide complex care coordination for members with Intellectual or Developmental Disabilities, focusing on Autism Spectrum Disorder in children. This remote role serves South Central or Mid State regions, with travel within the catchment as needed.

The position requires a qualified professional or licensed RN with MH/DD/SA experience, plus relevant master’s or bachelor’s level education.

Qualifications

• Must meet Qualified Professional (QP) status.
• Fully licensed by the NC governing board regulating Human Services professions; OR Fully licensed RN licensed in NC with MH/DD/SA experience.
• Graduate degree in a human service field with MH/DD/SA experience or substance abuse counseling experience; or a bachelor’s with MH/DD/SA experience; or equivalent.
• If LTSS needs, two years of LTSS/HCBS experience preferred.
• Must have a valid driver’s license and reside within Trillium’s South Central or Mid State region; ability to travel within catchment.

Responsibilities

• Serve as primary point of contact for assigned members with ongoing care management.
• Conduct comprehensive assessments using person-centered planning methods.
• Develop, implement, and monitor individualized care plans with members and families.
• Coordinate services across providers to address physical, behavioral, and social health needs.
• Ensure care plans reflect changes in member needs.
