Job summary

Hopscotch Primary Care is seeking a Care Manager Registered Nurse (CMRN) to oversee a panel of higher-acuity patients with a hybrid remote model and in-clinic presence as needed.

The role emphasizes end-to-end care management, reducing avoidable admissions, improving outcomes, and coordinating across providers, clinics, and community resources. Strong collaboration and communication are essential.

Qualifications

• Active RN license in North Carolina.
• BLS certification.
• Experience in a primary care clinic focused on chronic disease management.
• Experience with behavioral health and community-based organizations preferred.
• Experience with motivational interviewing, behavior change, health promotion, and coaching.

Responsibilities

• Panel Management & Care Coordination (Primarily - Remote). Manage a defined panel of high-risk patients and develop individualized care plans.
• Clinical Collaboration & Outcomes Management. Partner with providers and care teams to align care plans and track outcomes.
• In-Clinic Responsibilities (Hybrid). Maintain in-office presence at least once a month to support high-risk patient visits.
• Home & Community-Based Support. Coordinate home visits when clinically appropriate and assess social determinants of health.
• Patient & Family Engagement. Build trusted relationships and provide education on disease management and medications.
• Program Quality, Compliance & Best Practices. Adhere to care management protocols and contribute to process improvements.
