Qualifications

• Associates Degree required.
• BSN preferred.
• Compact state RN license required.
• At least 3 years post-acute nursing experience.
• Home health or hospice experience preferred.

Responsibilities

• Be a CenterWell Home Health representative for patients discharging or needing post-acute services.
• Provide patient options and education using multiple tools; address location lookup, directions, and complaints.
• Make clinical level of care determinations based on discussions and records; match needs to service sites or alternatives.
• Assist admissions by acting as an ambassador for patients meeting admissions requirements.
• Maintain awareness of performance objectives and ensure patient satisfaction goals are met.
