Molina Healthcare is seeking a remote RN Care Manager based in South Carolina to support care management and coordination across the continuum. You will collaborate with a multidisciplinary team to improve member outcomes while ensuring quality and cost-effective care.

This fast-paced, high-volume role requires comprehensive assessments, care planning, and ongoing monitoring, with travel as dictated by state requirements. Strong EMR/HIPAA knowledge and excellent communication are essential.

Qualifications

• Active SC RN license required.
• Minimum 2 years healthcare or care management experience.
• Valid driver's license and reliable transportation.
• Knowledge of EMR and HIPAA.
• Strong communication and problem-solving skills.
• Ability to work independently in varied settings.

Responsabilités

• Performs comprehensive member assessments within required timelines.
• Develops and implements care coordination plans with member and team.
• Conducts telephonic, in-person, or home visits as required.
• Monitors care plan effectiveness and documents progress.
• Maintains ongoing member caseload and outreach.
• Coordinates services including behavioral health and LTSS.
• Facilitates ICT meetings and collaboration.
• Uses motivational interviewing and Molina guideposts.
• Assesses barriers to care and provides assistance.
• May provide consultation to peers.
• May manage complex cases and medication reconciliation.
• Travel 25-40% based on state/contractual requirements.
