Job summary

Centene is seeking a Registered Nurse to develop and coordinate complex care management for primarily physical needs members in Missouri. The role supports pregnant members, with a Monday–Friday schedule, 8am–5pm, and the option to work remotely from home across the state.

You will evaluate needs, create care plans, connect with providers and resources, monitor outcomes, educate members and families, document activities, and ensure compliance with state and payer rules.

Qualifications

• Requires RN licensure and nursing degree (BSN preferred).
• 2–4 years of related experience in care management or a similar field.
• Ability to educate patients and families about care plans and resources.

Responsibilities

• Evaluate member needs, barriers to care, and social determinants of health.
• Develop ongoing care plans and coordinate with providers and resources.
• Identify problems and implement care management interventions.
• Coordinate between member, family/caregivers, and care team for person-centered care.
• Provide follow up, monitor status, and revise plans as needed.
• Provide education on disease processes, care options, and benefits.
