Job summary

Molina Healthcare is seeking a remote Care Manager with travel across Monroe County to support LTSS care coordination. You will work with a multidisciplinary team to ensure high-need members achieve desired outcomes and cost-effective care across the continuum.

Responsibilities include comprehensive assessments, waiver enrollment, and ongoing care plan monitoring, with emphasis on motivational interviewing and collaboration with providers and member networks. Travel up to 25-40% may be required.

Qualifications

• 2+ years healthcare experience, incl. LTSS, care management, or related setting.
• Licensure/certification required only if state contract mandates it.
• In some states, a health-related bachelor's degree may be required.
• Valid driver's license, reliable transportation, and auto insurance for travel.

Responsibilities

• Complete comprehensive member assessments within regulated timelines, including in-person home visits as required.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans in collaboration with members and care network.
• Monitor care plan effectiveness and document progress toward goals.
• Promote integration of behavioral health and LTSS services with community resources.
• Assess for medical necessity and authorize waiver services.
