Job summary

Molina Healthcare seeks a field-based care management professional to support LTSS activities and coordinate member care with a multidisciplinary team. This remote role requires travel within the greater Cook County area to perform assessments and home visits as needed.

You will develop and monitor care plans, ensure medical necessity, and collaborate with caregivers, physicians, and the ICT for service approvals while promoting cost-effective, quality member care.

Qualifications

• At least 2 years of healthcare experience, incl. care management and LTSS.
• Licensed social worker credentials (LCSW/LMSW/APSW/LPCC/LMFT or equivalent) active in state.
• Valid driver's license, reliable transportation, and ability to travel 25–40% locally.
• Ability to work independently with strong communication and problem-solving skills.
• Experience with community resources and coordinating across care teams.

Responsibilities

• Conduct comprehensive member assessments within regulated timelines, including in-person home visits.
• Facilitate comprehensive waiver enrollment and disenrollment processes.
• Develop and implement care plans with members, caregivers, physicians and care team to address needs and goals.
• Monitor care plans to evaluate effectiveness and adjust interventions as needed.
• Promote integrated services for LTSS and connect members with home/community resources.
