Qualifications

• Minimum 2 years healthcare experience including LTSS
• At least 1 year in care management or equivalent setting
• Valid driver’s license, reliable transportation, and travel readiness
• Ability to work independently and in diverse settings; strong communication

Responsibilities

• Completes comprehensive member assessments within regulated timelines, including in-person home visits.
• Facilitates waiver enrollment and disenrollment processes.
• Develops and implements care plans with members, caregivers, physicians and networks.
• Monitors care plan effectiveness and documents interventions.
• Promotes integration of LTSS and behavioral health to ensure continuity of care.
• Assesses medical necessity and authorizes waiver services; coordinates ICT meetings.
• 25-40% local travel may be required.
