Molina Healthcare is seeking a remote Care Manager/Coordinator to support long-term services and supports (LTSS) for high-need members. You will collaborate with a multidisciplinary team to coordinate integrated care, including home visits and waiver management, while advancing outcomes and cost-effective member care.

You will complete comprehensive assessments, develop care plans, monitor progress, and facilitate ICT meetings.

Qualificações

• At least 2 years of health care experience, including LTSS experience and care management.
• Clinical licensure and/or certification required only if mandated by state contract or licensing mandates.
• Bachelor's degree may be required in some states depending on contractual requirements.
• Valid driver's license and ability to travel.
• Proven knowledge of community resources.
• Ability to work independently with minimal supervision.
• Strong communication and organizational skills.
• Experience with individuals with disabilities or chronic conditions is preferred.

Responsabilidades

• Completes comprehensive member assessments within regulated timelines, including in-person home visits as required.
• Facilitates waiver enrollment/disenrollment processes.
• Develops and implements care plans in collaboration with stakeholders.
• Monitors care plan effectiveness and documents interventions.
• Promotes service integration across LTSS and behavioral health.
• Assesses medical necessity and authorizes waiver services.
• Evaluates benefits and funding sources.
• Facilitates ICT meetings for approval/denial of services.
• Uses motivational interviewing to support member change.
• Identifies barriers and coordinates psycho/social, financial, and medical support.
• Identifies critical incidents and develops prevention plans.
• Collaborates with licensed care managers as needed.
• 25-40% travel local may be required.
