Qualifications

• 2+ years health care experience including LTSS
• Licensure only if state contract requires it
• Bachelor's degree may be required in some states
• Valid driver's license and reliable transportation

Responsibilities

• Complete comprehensive member assessments within timelines, including home visits.
• Facilitate waiver enrollment/disenrollment processes.
• Develop and implement care plans with members and care team.
• Monitor care plan effectiveness and adjust interventions.
• Promote integration of services across LTSS and behavioral health.
• Assess medical necessity and authorize waiver services.
• Collaborate with care managers and leadership as needed.
• Travel 25-40% locally as required by contracts.
