Job summary

Molina Healthcare is seeking a field-based care coordinator in the United States with strong LTSS and care management experience. You will collaborate with a multidisciplinary team to coordinate integrated member care across settings, aiming for quality outcomes and cost-effective services.

Qualifications

• 2+ years health care experience with LTSS or care management experience.
• Valid driver’s license and ability to travel 25–40% locally.
• Experience with diverse populations and interdisciplinary teams.
• License required/optional depending on state contracts (if applicable).

Responsibilities

• Complete member assessments within timelines; include in-person visits as needed.
• Coordinate waiver enrollment/disenrollment processes.
• Develop and monitor care plans with members, families, and providers.
• Monitor care plan effectiveness and document progress toward goals.
• Promote integrated LTSS, behavioral health, and community resources.
• Assess medical necessity and authorize waiver services.
• Coordinate ICT meetings for service approvals and collaboration.
• Use motivational interviewing to educate and motivate members.
• Identify barriers and assist with psycho/social, financial, and medical needs.
• Identify incidents and develop prevention plans; collaborate with care managers.
• Travel up to 40% as required by state contracts.
