Molina Healthcare is seeking a patient-focused care coordinator to support LTSS-eligible members. You will collaborate with a multidisciplinary team to manage care across the continuum, ensuring progress toward outcomes and optimal utilization of resources.

The role involves comprehensive assessments, waiver coordination, care-plan development, and ongoing monitoring. Travel up to 25–40% may be required; strong communication and autonomy are essential.

Formación

• 2+ years health care experience including LTSS/care management experience
• Licensure/certification required only if state contract requires
• Bachelor's degree may be required in some states

Responsabilidades

• Complete comprehensive member assessments within regulated timelines; conduct in-person home visits as needed.
• Facilitate waiver enrollment/disenrollment processes.
• Develop and implement care plans with members, caregivers, physicians and support network.
• Monitor and evaluate care plan effectiveness; document interventions and outcomes.
• Promote integration of behavioral health and LTSS with community resources.
• Assess medical necessity and authorize waiver services; advise on funding sources.
• Coordinate interdisciplinary care team meetings for service decisions.
• Identify barriers and coordinate psycho/social/financial support for members.
• Travel up to 25-40% as required by state/contractual needs.
