Molina Healthcare is seeking a Care Manager to support LTSS care coordination for high-need members. You will collaborate with a multidisciplinary team to plan, implement, and monitor care across the continuum, ensuring member progress toward desired outcomes.

The role emphasizes comprehensive assessments, waiver coordination, and care-plan management. Travel of 25–40% may be required based on state contracts, with proficiency in Microsoft Office and strong communication essential for success.

Qualifications

• At least 2 years of health care experience with LTSS, care management, or related settings.
• Licensed/certified as required by state contract or licensing mandates.

Responsabilités

• Complete comprehensive member assessments within timelines, including home visits as required.
• Facilitate waiver enrollment/disenrollment processes.
• Develop and monitor care plans with members, caregivers, physicians, and care team.
• Assess and authorize waiver services based on medical necessity.
• Coordinate ICT meetings for service approvals and collaboration.
• Provide education and motivation during member contacts.
• Identify barriers and address psycho/social, financial, and medical obstacles.
