Qualifications

• 2+ years of healthcare experience, including LTSS, and 1 year in care management or equivalent.
• Valid and unrestricted driver’s license, reliable transportation for travel requirements.
• Demonstrated knowledge of community resources and ability to work across settings.
• Proficient in Microsoft Office and navigating online portals/databases.

Responsibilities

• Completes comprehensive member assessments within regulated timelines, including in-person home visits where required.
• Facilitates waiver enrollment/disenrollment processes.
• Develops care plans with members, caregivers, physicians and support network.
• Monitors care plan effectiveness and documents interventions/goal achievement.
