Qualifications

• At least 2 years of health care experience, including LTSS exposure, and 1 year in care management or related setting.
• Clinical licensure and/or certification required only if mandated by state contract or regulation.
• Bachelor's degree in a health care related field may be required in some states.
• Valid/unrestricted driver’s license and reliable transportation for job-related travel.

Responsibilities

• Complete comprehensive member assessments within timelines, including in-person home visits.
• Facilitate waiver enrollment/disenrollment processes.
• Develop and implement care plans with members and care team.
• Monitor care plans, document interventions, and adjust as needed.
• Promote service integration across behavioral health and LTSS.
• Assess medical necessity and authorize relevant waiver services.
• Coordinate ICT meetings for service approvals/denials.
• Travel 25-40% locally as required by state/contractual needs.
