Job summary

Molina Healthcare is seeking a remote, field-based Care Manager to support LTSS and waiver services across Marion and Mahaska Counties. The role involves travel and collaboration with a multidisciplinary team to ensure high-need members receive integrated, quality care.

You will complete assessments, coordinate service plans, and monitor progress, while addressing psycho/social and medical barriers to care. Strong communication and independent work skills are essential.

Qualifications

• Minimum 2 years health care experience with LTSS or related field.
• Clinical licensure/certification must be active if required by state/contract.
• Valid driver's license and reliable transportation for travel.
• Experience with waiver services and care coordination.
• Ability to work independently and in diverse settings.

Responsibilities

• Complete comprehensive member assessments within timelines, including in-person visits.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement individualized care plans with the care team and members.
• Monitor and adjust care plans to achieve member goals and outcomes.
• Coordinate services across behavioral health and LTSS for continuity of care.
• Assess medical necessity and authorize waiver services.
• Prepare for ICT meetings and coordinate with care managers.
• Identify barriers to care and assist with psycho/social obstacles.
