Job summary

Molina Healthcare is seeking a care management professional to support LTSS activities. You will coordinate member care across the continuum with a multidisciplinary team, including home visits and waiver services.

The role focuses on ensuring progress toward outcomes and alignment with quality and cost objectives. Responsibilities include timely assessments, waiver processing, care plan development, and ongoing monitoring.

Qualifications

• Strong verbal and written communication skills.
• Microsoft Office proficiency and ability to navigate online portals.
• Ability to work independently with minimal supervision.

Responsibilities

• Complete comprehensive member assessments within timelines and in-home visits as required.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members, caregivers, physicians and the care team.
• Monitor and adjust care plans to achieve goals and document outcomes.
• Coordinate LTSS with behavioral health and community resources.
• Assess medical necessity and authorize waiver services.
• Collaborate with the interdisciplinary care team and leadership as needed.
• Travel 25–40% for member visits as required.
