Job summary

Qualifications

• RN license active and unrestricted in state of practice
• Valid driver's license and ability to travel 25–40%

Responsibilities

• Perform face-to-face and telephonic visits with Medicaid members.
• Assess to determine services needed and develop care plans.
• Coordinate waiver enrollment and service delivery with the care team.
• Monitor and adjust care plans based on member progress and outcomes.
• Collaborate with ICT and other professionals to ensure integrated care.
