Job summary

Responsibilities include documenting visits within 24 hours, completing online training, and maintaining a flexible, work-from-home schedule with occasional travel for client visits.

Qualifications

• At least 1 year verifiable experience as a RN, LPN or Social Worker.
• Active professional license in your state.
• Good driving record and a reliable vehicle.
• Internet access for visit and assessment logging.
• Dependable person.
• No disciplinary actions against license or in excluded list.

Responsibilities

• Enter all assessment and visit information into the online system within 24 hours of the visit.
• Complete short online training session and webinar prior to the first visit.
• Visit the client once a week, 4 times a month on ongoing basis.
• Flexible schedule. Work from home.
