Job summary

Lancesoft is seeking a Care Manager I to support member engagement and care coordination in a fully remote role. The position requires a licensed social work or behavioral health professional with Nebraska licensure, and involves outreach, care coordination, and documenting activities in the case management system.

Qualifications

• Licensed social work or behavioral health professional eligible to practice in Nebraska.
• Minimum 5 years of social work experience.
• Strong communication, interpersonal, organizational, and documentation skills.
• Ability to work independently in a fully remote environment.
• Ability to handle Protected Health Information in accordance with HIPAA requirements.

Responsibilities

• Conduct outreach to educate members about needed healthcare services and coordinate appointments and resources.
• Conduct outbound calls to educate members about healthcare services and support engagement in care.
• Identify barriers to care and assist members in accessing appropriate services and resources.
• Schedule appointments with provider offices as needed.
• Connect members with appropriate community, healthcare, and support resources.
• Accurately document all outreach attempts, member interactions, outcomes, and follow-up activities in the case management platform.
• Maintain confidentiality and comply with HIPAA requirements and organizational policies.
• Collaborate with care management, provider, and quality teams to improve member engagement and help close gaps in care.
• Complete follow-up activities and maintain accurate and timely case documentation.
