Job summary

SCAN Health Plan is seeking a Remote Care Manager (LVN) to coordinate patient care with the Integrated Care Team. You will authorize services, review utilization, and ensure timely, appropriate care across settings. This role requires 3-5 years of related experience and a CA LVN license.

The position offers base salary with annual bonus, generous PTO, holidays, and comprehensive benefits, including retirement plans. Remote work mode is available and coverage includes after-hours.

Qualifications

• Associate's degree or equivalent required.
• 3-5 years of related experience preferred.
• CA LVN license required.

Responsibilities

• Serve as the point of contact with patients, families, and providers about care, transfers, and authorizations.
• Coordinate inpatient/outpatient services to ensure timely, appropriate care at the right location.
• Coordinate discharge planning from inpatient units and SNFs with post-acute needs.
• Ensure timely delivery of last coverage day letters to facilities.
• Develop and implement treatment plans using evidence-based criteria and clinical judgment.
